Some people are still unsure on this so figured a quick guide.
The portal requires 4 ingredients.
Eye of the beholder which is dropped by the boss in the halls of the elders which you kill for Harata - Strange Creatures aka second widget. Dc 35 lock for the easy way in by temple.
Caendryl gem is found on isle of death on (iirc) second floor as you approach the next locked door you continue down and the coffin thing at the wall has a transition which brings you to the crypts where you can fight the crypt warden. There are locked doors which are bashable.
Sendauntra medallion. This is found off the master of the dead in the dungeon of doors. In order to access this you must follow the open desert questline. for completing this questline you also receive a medallion.
Mana crystal. This is found on the isle of ice one the Crystal forest off enemy the ice troll. This is accessed after exiting the imass cave Where there is a transition to the left.
You may notice all are close to important quests excepting the medallion. So if you get a good party you can pick up the items as you quest.
With all 4 in hand go to caendryl and place the items in the corresponding pedestals. You will enter in the portalway. It's recommended to do a test on portals in the eye when you enter as the layout rotates each reset.

The main hub area of Sendauntra is called the eye of the spawn. When you leave any map you appear in the eye of the spawn. It has 8 portals in the shape of a clock face. These maintain the same layout however rotate each reset. For example. The portalway entrance is always opposite the npc area. If the npc area is south, portalway will be north. If the server then resets, the layout will rotate, but the portalway will still be opposite the npc area.

Also I have noticed I posted this in the build forum. Damn phone. Could you move it to strategys.
And yeah it's like an end game portal quest, and also access to some best end game monsters and a widget. The chest is cool too, using a key for a chest that is.

When you are getting the beholder part of the key in beholders from Salios' tomb, the area before entering that boss map is the one I refer too. Instead of going in and getting salios you go north and there is a dc 35 gate. This gate is with invis distance to the widget boss room. The DC is 35. If you haven't high enough open lock you need to take a left after the puzzle room and crawl through the tunnel. for a few areas.
